Title your Thread should follow this format:
Your Account Name - Ban Appeal

Your thread should include the following information:

Admin who banned you:
Reason for ban:
Date of ban (include time (with timezone) if known):

Please also include any other details you feel are relevant to your ban.

Please note: If you have nothing to do with the incident, weren't there when it happened, or don't know anything about it, do NOT post in the complaint about a player threads. This also goes for ban appeals.
